A Case of Kiloh-Nevin Syndrome

Abstract
Spontaneous paralysis of the anterior interosseous branch of median nerve is characterized by a sudden loss of power followed by paralysis of the muscles innervated by the nerve the flexor pollicis longus, the flexor digitorum profundus to the index finger and the pronator quadratus. Usually it i will be accompanied or proceded by pain in the region of the elbow of the affected side.
Paralysis of the pronator quadratus is not easy to detect but the limitation of pronation may be j easily formd due to the weakness of the affected forearm in the position of the arm held to the side of the trunk and elbows flexed to a right angle. The sensory changes are not usually found.
A case of Kiloh-Nevin syndrome developed in left hand was reported, who was 22 year old male,
with complaining of swelling, tenderness and pain of left arm with chest pain due to a simple fracture of the left humeral shaft which was treated by open reduction and fixed intramedullary Rush-pin through antero-lateral approach (Henry approach).
Post-operative 3 weeks, the physiotherapy was done, and at that time the patient complained
weakness of pronation of the left forearm. The physical examination revealed weakness of the flexor
digitorum sublimus and profundus of the index and middle finger without sensory change. After seven to eight months of active and passive exercises, the case was completely recovered. The following are important points through our experience.
1) The Flexor power of the index finger was an important criteria to be examined for Kiloh-Nevin syndrome.
2) Anterior interosseous nerve paralysis was more likely to occur with fracture of the humerus that was markedly displaced.
3) The injury was did not implicate the median nerve at the fracture site. 4) Spontaneous recovery in seven to eight months occured.
